EVALUATION SIMPLIFIED ACQUISITION PROCEDURES FAR PART 13
2.1 EVALUATION PROCEDURE
Subject line must include Solicitation Number, Project Title, Quoter Name.

COMPANY INFORMATION: UIE Number, Cage Code, Tax ID Number, General Contractor s License number and State of Licensure, principle points of contact with addresses, phone numbers and email addresses.
PRICE/COST SCHEDULE: Complete the SF 1442, Blocks 14, 15, 17, 19, 20A, 20B, and 20C to submit the Quote. All sections of the RFQ, excluding Instructions to Offerors and Evaluation Procedures Sections, constitute the model contract.
Quote with breakdown of labor, material, equipment, overhead and profit.
All contract line-item numbers in the price schedule must include a price.
The Government will evaluate quotes by adding all line-item prices - the total of all CLINS will be that sum. The Government will evaluate the price to determine whether it is considered fair and reasonable.
The Quoter shall submit the following information: (For a partnership or joint venture, the following submittal requirements are required for each Contractor who is part of the partnership or joint venture and for the entity itself. EMR shall not be submitted for subcontractors.)
OSHA or EPA Violation(s): Quoters shall provide Summary of Work-Related Injuries and Illnesses, affirming that the Quoters has no more than three serious, or one repeat, or one willful OSHA or EPA violation(s) in the past 3 years.
Experience Modification Rate (EMR): Quoters shall provide documentation to show that the Quoters has an Experience Modification Rate (EMR) of equal to or less than 1.0.
Submit completed certificate VAAR 852.219-75 VA NOTICE OF LIMITATIONS ON SUBCONTRACTING, attached to this solicitation.
EMAIL QUOTATIONS: The statement in block 13a. on page 1 of the SF1442 does not apply to this Request. Quoters shall submit their offer via email to donteana.gibbs@va.gov and aaron.holmes@va.gov by Monday, May 18, 2026, at 4:00PM ET. Only electronic offers will be accepted.
IMPORTANT: Exclusions must be clearly stated on the quote. Any questions concerning the solicitation shall be submitted by 10:00AM ET, Monday, May 4, 2026. Responses will be in posted as an amendment on Sam.gov.
BASIS FOR AWARD
The Government will evaluate the proposed quote to ensure it is fair and reasonable.
