INTENT TO SOLE SOURCE  The Department of Veterans Affairs, Network Contracting Office 4 (NCO4) intends to sole source with COSMED USA, LLC to provide a service contract for preventative maintenance and repair for existing COSMED Q-NRG+ machines at the Michael J, Crescenz VA Medical Center. The objective of this sole source award is for a Firm-fixed Price Contract, consisting of a base year plus four option years, with COSMED USA, LLC, 1850 Bates Avenue Concord, CA 94520, Unique Entity ID 020221102. This sole source determination is in lAW RFO 6.103-1(b) and RFO 12.204(a) utilizing simplified procedures. The North American Industry Classification System (NAICS) code for this procurement is: 811210, Irradiation Apparatus Manufacturing. The Small Business Administration (SBA) size standard is: $34.0 Million. The PSC code is: J065, Maintenance, Repair, and Rebuilding of Equipment Medical, Dental, and Veterinary Equipment and Supplies. This contract will be exempt from wage determination. The notice of intent to sole source is not a request for quotes.
