The following changes are made:
Addition of Attachment H Certification of Recall or Safety Issues
Attachment H (titled Certification of Recall or Safety Issues) is hereby added to the solicitation, Section D. Offerors shall complete, sign, and submit Attachment H with their proposals. Failure to submit Attachment H will result in the proposal being deemed Unacceptable.
Attachment A, Revision to Instructions to Offerors (Section E.5.B Volume I Technical)
The following language is added to Volume I Technical Factor:
Offerors shall submit Attachment H Certification of Recall or Safety Issues, signed by an authorized representative of the company, affirming that all proposed items have no outstanding recalls, have no known safety issues, and comply with all applicable medical equipment regulations and safety standards.
Attachment A, Addition to Technical Evaluation Approach (Section E.6.B Volume I Evaluation)
The following language is added under Volume I Technical Evaluation Approach:
Attachment H will be evaluated to confirm that all proposed items have no outstanding recalls or known safety issues. If Attachment H is missing, incomplete, or if the Government determines that any proposed item is subject to a recall or safety issue regardless of the Offeror s certification, the proposal will be evaluated as Unacceptable and removed from award consideration.
Attachment A, The following sentence in the Technical Evaluation Approach is revised to read:
For proposals meeting the salient characteristics provided in Attachment B and for which the proposed items have no known recall or safety issues the Government will conduct an evaluation using the technical subfactors listed above and assign an overall adjectival rating.
Attachment G, Industry Question Responses:
Industry questions and Government responses have been incorporated and posted as Attachment G_A0001.
FAR 52.222-90 Addressing DEI Discrimination by Federal (APR 2026) has been added under 52.252-2 CLAUSES INCORPORATED BY REFERENCE (FEB 1998).
