Award of Sole Source Contract for Healthcare Facilities Accreditation Services

NOTICE OF AWARD SOLE SOURCE NON-PERSONAL SERVICES CONTRACT

The Contractor provides all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform Medical Treatment Facilities accreditations and Patient-Centered Medical Home (PCMH) certifications. A Special Notice was posted in accordance with FAR 5.201, under ID # HT0011-25-R-0042 on November 24, 2025, with a completed J&A Sole Source on file.

Under the authority of FAR 6.302-1(a)(2)(iii)(A); Only One Responsible Source and No Other Supplies or Service Will Satisfy Agency Requirements.

The applicable NAICS Code is 813920, with a size standard of $23.5 million.

The applicable Product Service Code is R420.

Period of performance is 01 April 2026 through 31 December 2026, with four 12-month options to extend through 31 December 2030. The contract also includes the terms of FAR Clause 52.217-8 Option to Extend Services, which could further extend the period of performance through 30 June 2031.

A Justification and Approval, redacted in accordance with Federal Acquisition Regulation 6.302-1(a)(2)(iii)(A), is attached.
