The Indian Health Service, Four Corners Regional Health Center located at JCT US Highway 160 & Navajo Route 35, Red Mesa AZ 86514 has an upcoming requirement for a Blanket Purchase Agreement (BPA) for Triage D-dimer test kit supplies. Period of Performance will be a base plus four option years. The North American Industry Classification System or NAICS code is 339113 – Surgical Appliance and Supplies Manufacturing with a small business size standard of 800 employees. 

The Request for Quotations No. 75H71026Q00144 in accordance with the Simplified Procedures at Part 12.201-1 will be issued on or by May 1, 2026 12pm MST.  Evaluation factors may include technical capability to meet the government needs and price.  Quotes will be due on or by May 11, 2026. The acquisition is an Indian Small Business Economic Enterprise (ISBEE) set-aside.

In accordance with 52.204-7(b)(1) prospective contractors must have – and maintain (in accordance with 52.204-13(b) – an active profile in the System for Award Management at beta.SAM.gov in order to submit a quote.
