This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Requirements (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested, and a written solicitation will not be issued.
The solicitation number for this requirement is RFQ 75D301-26-Q-78995 and is hereby issued as a Request for Quote (RFQ) using FAR Subpart 13.1 Simplified Acquisition Procedures.
The Centers for Disease Control and Prevention intends to award a Time-and-Materials contract for NIOSH Medical Services - this is the 2nd revision to extend final date of proposals and pivot the requirement from a 5-year FFP to 3-year T&M contract for medical support services as described in the attached Statement of Work PDF, please either fill in the dollar amounts or provide a separate excel spreadsheet containing the same information including a list of line-item number(s) and items, quantities, units of measure, and options, if applicable:

Your quote shall include the following:
The quote format is: 1) A Technical proposal, which clearly identifies offerors capabilities to perform all required medical procedures in all 4 CDC continental USA geographic locations, including plans for mobile deployment and breakdown of anticipated Travel needs; and 2) A Pricing proposal, which itemizes cost per procedure per location for Government estimated levels utilizing the “Attach 4_T&M Pricing Template” excel spreadsheet, and include itemized estimate for Travel and any projected deployment costs.
It is the offeror's responsibility to be familiar with the applicable clauses and provisions. Clauses and provisions may be accessed via the Internet at website FAR | Acquisition.GOV. CDC and HHS have adopted the Revolutionary FAR Overhaul | Acquisition.GOV Deviations.
This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2026-01 03/13/2026.
This acquisition is set aside for total small business under North American Industry Classification System (NAICS) 621999 – All Other Miscellaneous Ambulatory Health Care Services – SBA size standard of $21M annual revenue. A Time-and-Material purchase order will be issued in writing to the successful offeror for a base year and two option years, for a total of 3-years possible. To be eligible to receive an award resulting from this solicitation, contractor must be registered in the System for Award Management at the time of quote submission, with the required NAICS code.
FAR Provision 52.212-1, Instructions to Offerors-Commercial Products and Commercial Services, applies to this acquisition.
FAR 52.212-2 Evaluation—Commercial Products and Commercial Services. (Deviation April 20, 2026)
(a) Evaluation factors. The Government will award a contract resulting from this solicitation to the responsible Offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors will be used to evaluate offers:
CDC intends to award a Time-and-Material contract from a total small-business (SB) set-aside for a base year of performance plus two-option years. SB proposal will be assessed for technical acceptance in providing ability to perform the required medical testing at the designated geographic location on a Lowest Price Technically Acceptable determination. Price will be assessed for fair and reasonable, providing best value to the Government. CDC will be looking for discounts from open-market pricing.
INSTRUCTIONS TO OFFERORS
1. LPTA – Lowest Price Technically Acceptable. CDC will evaluate all quotes to include the proper medical procedures and processes, and adequate description of all travel and deployment plans to justify pricing and stated capacity to meet all 4 CDC geographical place of performance requirements. Once deemed technically acceptable, CDC will make an award to the lowest price for annual estimated fulfilment of NIOSH Medical Service needs as stated in the SOW.
2. Offerors are also required to submit a pricing proposal for all estimated tests pricing per year, can be submitted as separate documents or by filling in the estimated “NOT TO EXCEED” annual value for CLIN 0001, 1001, 2001. Travel CLIN 0002, 1002, 2002 should be estimated travel values to perform the quantity of test listed in the SOW.
3. Proposal Due Date: via email to wlv7@cdc.gov – Monday, July 20, 2026, No Later Than 3:00 PM ET. NO LATE PROPOSALS will be accepted, please request confirmation of receipt to confirm timely submission.
4. Questions and Answers: 5 Questions were submitted and Answers are posted as attachment: “RFQ 75D301-26-Q-78995_Q_A”. One answer was edited with the change to T&M and the inclusion of “Attach 4_T&M Pricing Template”. Any additional questions must be received via email to wlv7@cdc.gov no later than 3:00 PM ET on Tuesday, July 14, 2026.
(b) Options (if applicable). The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. The evaluation of options does not obligate the Government to exercise the option(s).
(c) Notice of award. A written notice of award or acceptance of an offer furnished to the successful Offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer’s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
