Attending Veterinarian

This procurement is for veterinary services in accordance with the request for quotation (RFQ) and the statement of work (SOW).  Quotation documents must be compatible with Microsoft Word or Excel or viewable in Adobe Acrobat. Documents must not exceed 5 megabytes in total file size.

Unique Entity Identifier (UEI)
To document compliance with FAR 52.204-7(b)(1), your quotation response MUST include your organization's unique entity identifier (UEI) from www.SAM.gov.

Award Without Discussions
The Government intends to evaluate quotations and award without discussions with offerors, but reserves the right to conduct discussions, if the Contracting Officer determines them to be necessary. The offeror’s initial proposal should contain the offeror’s best pricing and delivery, plus include a complete but concise technical quotation.
