A. SOLICITATION INFORMATION
This is a Combined Synopsis/Solicitation for commercial Preventive Maintenance (PM) Services for Government-owned Optometry Equipment prepared in accordance with the applicable Federal Acquisition Regulation (FAR) commercial acquisition procedures. This announcement constitutes the only solicitation. Quotations are being requested in response to this notice, and a separate written solicitation will not be issued.

Solicitation No. 75H70726Q00092 is issued by the Department of Health and Human Services (HHS), Indian Health Service (IHS), Albuquerque Area Office (AAO). 

This solicitation is issued as a Total Small Business Set-Aside. Only quotations submitted by responsible Small Business concerns will be considered for award.

This solicitation incorporates the provisions and clauses in effect through Federal Acquisition Circular (FAC) 2026-01, applicable Health and Human Services Acquisition Regulation (HHSAR), and applicable class deviations.

Content of Quotation Submission
To be considered for award, each Quoter shall submit the following by the quotation due date and time specified in this solicitation:
1.    Technical Qualifications (Volume I)
2.    Past Performance (Volume II) (Past Performance Survey Template attached)
3.    Price Quotation (Volume III)
4.    Signed Standard Form (SF) 1449
5.    Acknowledgment of all solicitation amendments, if applicable

Question Submission
Questions regarding this solicitation shall reference Solicitation No. 75H70726Q00092 and be submitted using the attached Question and Answer (Q&A) template, no later than  August 13, 2026,  at 12:00 p.m. Mountain Standard Time (MST).

Responses to timely submitted questions will be provided through a solicitation amendment, as applicable.
