Purpose
The purpose of this Statement of Work (SOW) is to define the requirements for procuring medical grade instruments to support dermatologic surgical procedures performed in the MOHS Clinic at the Ralph H. Johnson VA Medical Center. These instruments are essential to ensure safe, sterile, and efficient procedural operations for the diagnosis and treatment of skin cancers.
Background
The MOHS Clinic performs tissue-sparing surgical procedures that require precision, sterility, and specialized instrumentation. Current equipment is either outdated, incomplete, or insufficient to meet procedural demands. Updated medical grade instruments are required to maintain clinical readiness, support patient safety, and comply with VA, Joint Commission, and infection control standards.
Scope of Work
The contractor shall provide medical grade surgical instruments suitable for dermatologic and MOHS micrographic procedures. Items must meet all specifications outlined in Section 5 and be compatible with VA sterile processing (SPS) protocols.
Deliverables
The contractor shall provide ALL of the following deliverables
Delivery of specified instruments.
Manufacturer documentation for cleaning, sterilization, and maintenance.
Warranty information.
Replacement or correction of any damaged or non conforming items upon receipt.

Meet or exceed AAMI and FDA medical device standards.
Be surgical quality, precision engineered for dermatologic procedures. The vendor shall ensure all item numbers, quantities, and models are consistent with the associated quote and procurement request.
Deliverables
The contractor shall provide ALL the following:
Full instrument set as listed in the finalized bill of materials.
Itemized packing list upon delivery.
Manufacturer cleaning & sterilization instructions for SPS.
Warranty documentation and point of contact information.
Delivery Schedule
All items shall be delivered within 30 calendar days of contract award, unless otherwise approved by the Contracting Officer (CO). Early delivery is acceptable.
Acceptance will be performed by MOHS Clinic leadership and SPS. Items will be inspected for:
Conformance to ordered specifications
Structural integrity and absence of defects
Full functionality
Completeness of order
Any non conforming or damaged items must be replaced at no additional cost.
Security Requirements
No on site installation or system access is required. No patient information will be accessed or handled.
Warranty and Support
The contractor shall provide standard manufacturer warranties. Defective instruments must be replaced promptly at no additional cost during the warranty period.

(a)Definitions. As used in this clause
Program participation means membership or participation in, or access or admission to: training, mentoring, or leadership development programs; educational opportunities; clubs; associations; or similar opportunities that are sponsored or established by the contractor or subcontractor.
Racially discriminatory diversity, equity, and inclusion (DEI) activities means disparate treatment based on race or ethnicity in the recruitment, employment (e.g., hiring, promotions), contracting (e.g., vendor agreements), program participation, or allocation or deployment of an entity's resources.
(b) In connection with the performance of work under this contract, the Contractor agrees as follows:
(1) The Contractor will not engage in any racially discriminatory DEI activities;
(2) The Contractor will furnish all information and reports, including providing access to books, records, and accounts, as required by the Contracting Officer, for purposes of ascertaining compliance with this clause;
(3) In the event of the Contractor's or a subcontractor's noncompliance with this clause, this contract may be canceled, terminated, or suspended in whole or in part, and the Contractor or subcontractor may be declared ineligible for further Government contracts;
(4) The Contractor will report any subcontractor's known or reasonably knowable conduct that may violate this clause to the Contracting Officer and take any appropriate remedial actions directed by the Contracting Officer; and
(5) The Contractor will inform the Contracting Officer if a subcontractor sues the Contractor and the suit puts at issue, in any way, the validity of this clause.
(6) The Contractor recognizes that compliance with the requirements of this clause are material to the Government's payment decisions for purposes of 31 U.S.C. 3729(b)(4).
(c) The Contractor must include the substance of this clause, including this paragraph (c), in subcontracts at any tier, including those for commercial products and commercial services, except those where the place of delivery or performance is outside the United States.

A comparative evaluation process will be completed to determine the awardee.

All quoters must provide the Country of Origin (COO) for ALL line items.
A comparative evaluation will be completed in accordance with FAR 12.203.
All quoters must clearly meet or exceed all stated salient characteristics.
Al quoters must provide, with their quotation, a cross reference to clearly show the item(s) quoted meet or exceed all stated salient characteristics.
Quotes found non-compliant with the solicitation will be removed from further consideration.
The Government will award a single contract resulting from this solicitation to the responsible quoter whose offer conforms to the solicitation and will be most advantageous to the Government, price and other factors considered.
