SAMPLE BASIC MILITARY TRAINING (BMT) METAL NAME TAGS SAMPLE SUBMISSION. 

This Special Notice is issued to provide specific instructions to potential quoters for the mandatory pre-bid submission of six (6) Metal Name Tag samples as follows:

• Four (4) USAF samples engraved with the names, CARTER, and BINET-IRIZARRY with the USAF color Pantone 2747C.  Names must be all caps.
• Two (2) USSF samples engraved with the name HAILEY LOCKETT with the USSF color Pantone 2188C.  Names must be all caps.

The address for where the samples must be sent is included in the attached Statement of Work. Additional details regarding this Special Notice is also attached.
